Case Manager/Mental Health - Middletown Outpatient

The organization offers evidence-based treatment for adults and children with serious mental health diagnoses, substance abuse problems and co-occurring disorders.
CPC is hiring an outpatient case manager who will work as a member of an interdisciplinary team responsible for coordinating services for and working with SMI consumers and their support network to promote wellness, recovery and productive lives. The care manager will work with the consumer and treatment team in planning and implementing goals in order to maximize consumer's quality of life.
Responsibilities include completion of all documentation for outpatient clients including progress notes and treatment plans. Case managers will provide client centered care management services by removing potential barriers to treatment and arranging services (e.g., transportation, housing, financial support, etc.) needed on an individual client basis as per the treatment plan. Care managers will coordinate and collaborate with clients, their families, and outside providers.
Bachelor's degree in mental health or related field with 1 (one) year of experience; 2 (two) years is preferred. A valid NJ driver's license with a satisfactory driving history is required.
